Rehabilitation Unit


Ambassador Healthcare specializes in Rehabilitative Care.  Whether you or a loved one has suffered from the effects of a stroke, had a joint surgery or replacement, have had an injury or are simply in need of general short-term physical rehabilitation, Ambassador Healthcare has the service to fit your individual needs.

Our dedicated and highly skilled staff offers physical therapy, speech therapy and occupational therapy supervised by a rehabilitation doctor.  Each resident receives an individualized careplan designed specifically for the needs of that resident.  The careplan consists of activities and goals focused on restoring the resident’s independence and returning them home as quickly as they are able.

Unlike other rehabilitation units in our area, our Rehab to Home residents may choose to stay in a PRIVATE room.  These are available on a first come, first served basis.  They are available to private pay residents, Medicare residents AND Medicaid residents.

Alzheimer’s/Memory Care Unit


Ambassador Healthcare’s Alzheimer’s/Memory Care Unit is the finest in the Centerville area because of our dedication to fostering an atmosphere of respect, dignity and resident safety, all under the umbrella of Ambassador’s reputation for first-class, resident-focused, compassionate care. We offer:
  • Constant supervision.
  • Formal nursing care plans specifically designed for each resident.
  • Assistance with daily living activities.
  • Personalized care designed to offer residents both mental and memory stimulation.
  • Dedicated activity staff who provide DAILY programs and activities.
  • Secured outdoor walking area within our beautiful wooded campus.
  • A certified Alzheimer’s director.

  • A dignified wandering control program.
  • A dedicated psychiatric medical director, Pravesh Patel, M.D. (we are the only facility in Wayne County with such a position!).

Ventilator Independence Unit


Ambassador Healthcare leads the state in weaning ventilator residents from dependence on their vent and their trach. That is why we refer to our unit as the Ventilator Independence Unit.

Ambassador Healthcare’s ventilator unit is unparalleled in its commitment to the highest quality of care, and that dedication is reflected in Ambassador’s rate of success weaning patients off their vents and trachs, returning them to a more normal quality of life.

Although not every person can be weaned from the vent, Ambassador’s staff has worked hard to earn a reputation of weaning many patients originally deemed unweanable.  Since January, 2011, when the unit opened, over 100 residents (as of April, 2016) entered our unit vent dependent and left weaned from their vent AND their trach, having regained a higher quality of life and independence.
There is no secret to our success weaning vent and trach patients. It starts with a philosophy and attitude that fosters an environment of success. Add to that the dedication, skill and experience of our staff, and the result is a reputation for weaning that is second to none.

In Addition, Our Ventilator Independence Unit Offers:


  • Respiratory therapists on-site 24 hours a day.
  • Experienced pulmonologist Dana Reihman, M.D. 
  • Portable ventilators allowing our residents increased mobility and independence.
  • Daily rehabilitation and physical therapy.
  • 6.4 staff hours per patient, per day – a level of care that’s unheard of in the industry.
  • Pulmonary rehabilitation.
  • Our vent unit is QRCR certified by the American Association of Respiratory Care.

Pulmonary Rehabilitation


Advanced Lifeline Services, Inc. (ALS) specializes in providing ventilator and respiratory care services to individuals on life support systems. The ALS ventilator program is designed specifically for long-term care facilities and dedicated to provide the best possible care and outcomes for the individual. Each ventilator unit involves an interdisciplinary team of physicians, nurses, respiratory therapists, nutritionists, social workers and other staff members to contribute their expertise. ALS ventilator and respiratory care services provide:

Long-Term Acute Care

In addition to nurses, aides and therapists, Ambassador is the only facility in the area with three physicians on staff.  Dr. David DeSantis, M.D. is our Medical Director.  Dr. Dana Reihman, M.D. is our Pulmonologist.  Dr. Pravesh Patel, M.D. is our Behavior Specialist/Psychiatrist.  Having three expert physicians on staff is just another way Ambassador insures that our residents receive the best care available.
